To add a question to the form, select Question Text from the Question element drop-down list.  Then from the question type drop-down list choose the question type that is suitable for your requirements.

 

 

The window will then be configured for the text you need to enter to compose your question.

 

You can add the following types of questions to the form:

 

Multiple Choice - Single Response: The option to choose only one response from 2 or more options

Multiple Choice - Multiple Response: The option to choose any number of responses from 2 or more options.

Rating Scale - Single Question: A single rating scale question with an optional follow on question

Rating Scale - Question Block: One or more rating scale questions presented in a block

Commentary: Text comments provided by the respondent.
