A form has the following properties most of which you can configure from the Properties window. See Changing form Properties for more information.
Status
Indicates whether a form is locked or unlocked. Before respondent information can be entered against a form it has to be locked.
If you import a form from the template library or from another project, it is locked by default. However if you create a form from scratch you will need to explicitly lock it. See Locking and Unlocking forms for more information.
Type
Provides standard name tags for forms that is independent of any naming convention you might use. See Working with form Types for more information.
This is blank by default and is an optional field.
Issue Date
The date on which the form was issued. This allows you to keep track on when forms were handed out to respondents.
This is blank by default and is an optional field.
Respondents
This is a count of responses that is counted up as you add responses to the form. You cannot directly change this number as it is calculated from the responses that you enter.
Response Rate
This calculates the response rate based on the number of delegates you entered in the evaluation details section and the number of responses that have been entered.
In certain circumstances, a response rate will not be meaningful - for example when an instructor fills out an evaluation form. In this instance you can set the property to not calculate this rate.
By default, the response rate is set to be calculated.
