You can use the designer to create template or project forms.

This icon identifies a template form. A template form is one that is stored in the template library and can be copied into projects as required. You will normally create one or more of these that can be reused in different evaluation projects. You can also setup benchmark scores that will be saved along with the form.

This icon identifies a project form. A project form is a form that has been assigned to a project. You can either explicitly create a project form or import a template form into an evaluation project.

To create a new form you can either design it from scratch or open an existing form, save it under a different name and then modify it to suit your requirements.
