The selected evaluation form is converted into a Microsoft Word form and then attached to the e-mail which is then sent. This format makes it easy for your respondents to complete and return by email where they have at least Microsoft Office 2000 installed on their computer.
You have 2 options with regards the attachment:
You can have Trellis create the Microsoft Word evaluation form and send it as a single operation
Or
You can export the form as a Microsoft Word form for review and use Trellis 2011 to attach it when you click send.
Automatically Attach the Evaluation Form
This is the default selection as below:

You can choose to change the name that will be used to save the Microsoft Word form. If you do not change it, it will have the same name as the selected form.
Manually Attach the Evaluation Form
Click on Attach it Manually, the attachment area will then look like this.

Before you can attach the from manually, you need to export it to a location in your file system first. To do that, click on the export button at the top of the window

Click on the Export Evaluation Form button, this will pop up the save file dialog

Enter the name you want to use to save the Microsoft Word form or accept the default name which will be the name of the form that you are sending.
Click the Save button and the Microsoft Word form will be created and saved in the location you specified. You can then open the form using Microsoft Word and review it.
The form is protected by default to prevent users changing it or corrupting it in any way however it is not locked with a password. You can easily unlock the form and make any changes you might need to make and in addition save it with a password to ensure no one makes any alterations to it.
Once you are happy with the Microsoft Word form and are ready to send it to your delegates, specify the path to it by clicking on the ellipsis button
by the Enter Attachment Path text box. When you click Send, Trellis 2011 will attach the form picking it up from the location you specify however it will not attach a form that does not correspond to the selected form.
IMPORTANT:
Whilst you can make changes to the Microsoft Word evaluation form you are advised not to delete any of the controls from the form or add any to it. Making these types of alterations may make it impossible for Trellis 2011 to read the form when it has been completed and returned.
In a case where Trellis 2011 cannot recognise the form as something it created, you will have to enter the results manually.
