Rating Scale - Single Question

Rating Scale - Single Question presents the respondent with a question which they are expected to respond to using a grading scale. It provides the ability to optionally add a follow on question.
Adding a Rating Scale - Single Question
Select Question Text from the drop down box and then choose Rating Scale - Single Question. The form designer will then be configured as follows:

Enter the text of your question at the Enter Question Text prompt.
To add the Rating scale that you want the respondent to use to rate the question subject, click on the Add a Rating Scale Group button
. See Working with Rating Scale Groups for more information on adding a rating scale groups to the form designer.
With question text and the rating scale group updated, you can choose whether you want to ask a follow up question for example "Please explain why?"
Depending on the nature of the question, the space requirements for the response will vary. In order to specify how much space should be provided on the form, set an appropriate value for Number of lines for response.
To choose whether to number the question or not, you can check or uncheck Number this Question. You can change this setting at any time if you change your mind by editing the question and making the relevant change.
By default your question will be added to bottom of the form. However if you want to place the question directly into position you can use Relative Positioning. See Working with Relative Positioning for more information.
Even if you choose to work with the default, you can move the question into place by using the Move up or Move down options. First select the item you want to move and then:
From the menu select Modify Layout -> Move Up/Move Down
or
Right Click and then select Move Up/Move Down
or
From the menu select the relevant icon
or
With all your selections complete, click on the Add to form button and your question is added to the form. You can see the text of the question in the layout panel which you can also drill down to see the full details of the question.

Editing a Rating Scale - Single Question
To edit a Rating Scale - Single Question, first select the question in the layout panel then:
From the menu select Modify Layout -> Edit
or
Right click and select edit from the shortcut menu
or
Select the edit icon from the toolbar ![]()
This window will open with the text of the question presented for edit:

Make any changes as required to the question text. To edit the Rating Scale Group items, click on the Edit the Rating Scale Group button

Once you are satisfied with your changes, click OK. Your form will be updated to reflect your changes. You will also see the changes reflected on the layout panel.
Deleting a Rating Scale - Single Question
If you decide to remove a Rating Scale - Single Question from your form, you can delete it by first selecting it and then:
From the menu select Modify Layout -> Delete
or
Right click and then select Delete
or
Click on the delete icon in the toolbar ![]()
You will prompted to confirm your action. Once you confirm that you indeed want to delete the question, it will be deleted. You will be unable to undo the delete action once it has been performed.
