To create a form from scratch you need a blank form to which you can add the elements that you require for your form.

 

When you first open up the form designer, a blank form is opened by default.  If you need to create a new form:

 

 From the menu bar select form ->

 

or

 Click on the New form icon .

 

 

A blank form will have a temporary name of Untitledx where x is a number that is just counted up. You can change the the name at any time
