To change a forms properties,  first select it then:

 Click on the Change Properties icon on the left panel

or

 Right-click and the select Change Properties from the shortcut menu.

 

 

The following window will then open

 

 

 

Form

 

This is the name of the form you selected.

 

Status

 

Check this box to lock the form and uncheck it to unlock it.  By locking the form you disallow changes to be made to the form or its benchmarks.

 

Evaluation Type

 

This presents a drop-down list that describes focus of evaluation form.  If a evaluation type is not in the list, you can add it by selecting Add a New Evaluation Type from the the drop down list.  See Working with Evaluation Types for more information.

 

Issue Date

 

This presents a drop down calendar from which you choose the date on which you issued the form to respondents.

 

Response Rate

 

Check this box to indicate that you want to have the response rate calculated.  This is the default setting however if you do not want a response rate to be calculated, uncheck this box.

 

 

When you are satisfied with your changes, click on the Save button and your changes will be saved. The forms panel will be updated to reflect your new properties.
