To add a Question Block to a form,  click on the Add a Question Block button , the Add a Rating Scale Question Block window will then open.

Adding the block to the form

 

Once you have created the question block, you can choose to save to it to the library first or just copy it directly to the form designer.

 

To save the newly created block to the library, read the section on Saving to the library here Saving a Question Block to the Text Library

 

To copy the group to the form designer, click on the OK button, the editor window will then close and you will see your options in the list box in the designer.
