To add a Multiple Choice - Multiple Response Group to a form,  click on the Add a Multiple Choice Group button , the Add Group  editor will open.

Adding the group to the form

 

Once you have created the group, you can choose to save to it to the library first or just copy it directly to the form designer.

 

To save the newly created group to the library, read the section on Saving to the library here Saving a Multiple Choice -  Multiple Response Group to the Text Library

 

To copy the group to the form designer, click on the OK button, the editor window will then close and you will see your options in the list box in the designer.
